He is wearing white, black and red make-up. When you see him as the police officer in the assassination scene, that is the character sans face. If he were perma-white, how come it gets all smeared and rubbed off during his arrest? (and is then restored when he meets Dent in the hospital room).

We’ll have to wait for the DVD, but I got the distinct impression from The Dark Knight that the Joker was wearing flesh-colored make up when disguised as a police officer. His face only appears for a second, but it looks artificially colored. Perhaps someone who has seen the film more than once could chime in?

I believe its the scene where they “kill” Gordon. The Joker was dressed as one of the Guards on stage, and in one quick shot you see his face, the scars are visible but there is no make-up.

I took this, as well as his revisionist autobiography, to say that he willing chose to become the Joker. No one made him this way. He gave himself the scars, and a series of horrifying stories about them, and the face paint and hair color, and costume. So i think your conclusions about him are correct. He’s only trying to amuse himself. He is simply preying on people’s fears. Why would anyone destroy their own face? is nearly the same question as Why would anyone blow up a hospital for the sake of blowing up a hospital? It makes him all the more nightmarish.
